CUSTOMER DETAILS & INFORMATION

1. All details and information are only for administration purposes and not to be used or exposed for third-party viewing.

2. Your details are only recorded via Consultation Form and Artistry 2.0 application. If there is any additional forms that is not mentioned in the above, we will require your further consent before proceeding.

3. Customer must be present to fill in their details. Second or third-party cannot fill in the details unless it is consented. 

4. You are allowed to leave out any details that you deem is sensitive or of no importance, unless it is stated as a requirement (*).
